Thank you for your interest in enrolling your child at L. Hollingworth School
As a charter public school, we are pleased to offer a tuition-free K - 8th educational choice. Enclosed you will find a student registration packet. This packet contains very important documents, including permission forms, special education placement forms, medication notification, and emergency procedure information. Completing the enclosed packet in its entirety will advance your child to the next stage of the enrollment process.
Please print and read these forms carefully, complete them thoroughly, and return this package to the school.

Attention Incoming Kindergarten,1st, 2nd, and 3rd-Grade Parents!
Kindergarten, 1st, 2nd, and 3rd-Grade openings are limited, and enrollment is not guaranteed.
Submitting an enrollment application does not guarantee placement in our Kindergarten through 3rd-Grade program. Availability is based on the number of openings.

Your child's enrollment is not complete until all required documents have been submitted and verified. Please upload the following documents to complete the enrollment process for Ohio public schools:
- Child's Birth Certificate (birth record from hospital will NOT be accepted) Please note: Your child must be 5 years of age by the first day of the current school year to enroll in Kindergarten.
- Two (2) Proof of Residency (any utility bill, bank statement, or paycheck stub. Must show the address and a current date)
- Valid Photo ID for the parent or legal guardian
- Custody or Guardianship Documentation (if applicable)
- Completed Physician's Report (completed and signed by the child's physician)
- Immunization (Shot) Records for the student
- Dental Examination Form (Kindergarten students only)
- Student's Most Recent Grade Card
- Most Recent State Test Scores (if applicable)
- Current IEP, ETR, and/or 504 Plan (if applicable)
- Discipline records, such as suspension and/or expulsion Records (if applicable)
