K-8 New Student FinalForms Enrollment Process
REQUIRED DOCUMENTS CHECKLIST FOR THE ONLINE ENROLLMENT PROCESS
To complete the enrollment application in FinalForms for students in Kindergarten through 8th grade at L. Hollingworth School, parents and guardians must upload the required enrollment documents. Please ensure that all necessary documents are available and ready for upload before beginning the registration process for your new student.
- Child's birth certificate (birth record from hospital will NOT be accepted) Please note: Your child must be 5 years of age by the first day of the current school year to enroll in Kindergarten.
- Two (2) Proofs of Residency such as a lease, rent receipt, utility bill, JFS, Buckeye Cable bill, pay check stub, or W- 2 Forms. (must match Parent/Guardian name and address on application)
- Parent/Guardian Photo I.D.
- Immunization Record
- Physicians Report Please click the link to print out the Physicians report: LHS Physicians Report, or you may pick one up in the main office.
- Dental Screening (Kindergarten only) Please click the link to print out the kindergarten dental form: Kindergarten Dental Form, or you may pick one up in the main office.
- IEP / ETR / 504 - if applicable
- Grade Card - most current
- State Test Scores (Grades 3-8)
- Discipline Records (suspensions, expulsions, etc.)
- Custody Papers - if applicable
- $20 non-refundable school/materials fee - Please pay during LHS Open House or the first week of school.
The school has established a standardized testing program and designated minimum standards acceptable for the early admission of a child to kindergarten. On request of the parents or guardian the school shall provide testing to children who will be 5 years of age on or before the first day of January for which admission is requested, and shall admit the children who pass such tests.
